世界の平均寿命がパンデミック前の水準へ急回復、しかし健康寿命の遅れと慢性疾患の台頭が新たな課題に

世界保健機関、いわゆるWHOが最新の世界保健推計を発表しました。
新型コロナウイルスの世界的な流行によって、人々の寿命は大きな影響を受けました。
しかし、最新のデータによると、世界の平均寿命はパンデミック前の水準へほぼ回復していることが明らかになりました。
具体的には、2023年の世界の平均寿命は73.3歳に達しました。
これは、パンデミック前の2019年に記録された73.4歳に極めて近い数値です。
WHOのデータ・デジタル技術・分析・AI部門のディレクターは、寿命が延びたことは公衆衛生における偉大な成果の一つであると述べています。
People around the world are living longer again, with global life expectancy almost fully recovered from the disruption of the COVID-19 pandemic, the World Health Organization (WHO)saidon Friday.
According to the UN health agency’s latestGlobal Health Estimates,life expectancy reached 73.3 years in 2023, just shy of the 73.4 years recorded in 2019.
“Living longer is one of the great achievements of public health,” said Dr. Alain Labrique, Director ofWHO’s Department of Data, Digital Health, Analytics and AI.

今回の推計は、2000年から2023年までの世界的な死亡と障害の傾向を分析したものです。
分析にあたっては、各国の人口動態登録データや、WHOの技術プログラム、さらに国連のパートナー機関による推計など、多様な情報源が用いられました。
一方で、健康に暮らすことができる期間を示す「健康寿命」の回復は、平均寿命に比べて遅れていることが示されています。
2023年の健康寿命は62.8歳となっており、2019年の水準と比べて0.4歳下回っています。
長生きを達成した次の課題は、増えた寿命をいかに健康に過ごすか、そして変化する人口ニーズに医療システムがどう応えるかだと指摘されています。
The estimates, which cover trends in death and disability worldwide by region and by country from 2000 to 2023, draw on various sources including national vital registration data and estimates from WHO technical programmes alongside UN partner organizations.
Healthy life expectancy –the years people live in good health – has recovered more slowly, reaching 62.8 years in 2023, still 0.4 years below its 2019 level.
“The next challenge is to ensure that those additional years are lived in good health, while health systems are equipped to respond to the changing needs of populations,” Dr. Labrique said.

ここからは、今回の推計から明らかになった、世界の死因に関する3つの重要な変更点についてお伝えします。
1つ目の重要な変更点は、低所得国における死因の構造変化です。
2023年、低所得国において感染症による死亡者が、全死亡者数の半分未満となりました。
これは歴史上初めてのことであり、WHOが「世界的疫学移行」と呼ぶプロセスの進展を示しています。
世界全体で見ると、2023年の死亡者の74%が非感染性疾患、いわゆる生活習慣病などの慢性疾患によるものでした。
この割合は、2000年の58%から大幅に上昇しており、世界の10大死因のうち8つを非感染性疾患が占めています。
特に心血管疾患は依然として最大の死因であり、虚血性心疾患だけで2023年に約950万人の命が失われ、2億1000万年分の健康寿命が失われました。
この負担を減らす取り組みは、2000年以降、世界の多くの地域で大きな進歩を遂げました。
しかし、西太平洋地域や東南アジア地域では個人のリスクが増加しており、根強い不平等が存在することが浮き彫りになっています。
In 2023, for the first time, communicable diseases – one that spreads from one person or animal to another, or from a surface to a person –accounted for less than half of all deaths in low-income countries, a further step in what WHO calls the “global epidemiological transition”.
Worldwide,noncommunicable diseases (NCDs) caused 74 per cent of deaths in 2023, up from 58 per cent in 2000, and eight of the world’s 10 leading causes of death were NCDs.
Cardiovascular diseasesremain the leading cause of death and disease burden. Ischaemic heart disease alone caused approximately 9.5 million deaths in 2023 and the loss of 210 million years of healthy life.
Many parts of the world have made substantial progress in reducing that burden since 2000, though individual risk increased in Western Pacific and Southeast Asia regions, highlighting persistent inequalities.

2つ目の重要な変更点は、認知症や糖尿病といった慢性疾患による死亡リスクの急上昇です。
認知症は、2000年には世界の死因の19位でしたが、2023年には5位にまで浮上しました。
この期間中に、認知症による死亡者数は3倍に増加しています。
また、糖尿病による死亡リスクも2000年以降、大幅に上昇しています。
この糖尿病のリスク上昇は、特に東南アジア地域において顕著に見られます。

The risk of dying fromdiabeteshas also risen substantially since 2000, particularly in South-East Asia.

3つ目の重要な変更点は、精神疾患による健康寿命の損失と、薬物使用障害における地域的な格差です。
2019年から2023年の間に、精神的な要因で失われた健康寿命の割合が増加しました。
年齢調整された健康寿命の損失割合は、うつ病によるものが約20%増加しました。
さらに、不安障害による損失割合は、同じ期間に約45%も増加しています。
また、薬物使用障害については地域ごとの差が非常に顕著です。
2000年から2023年にかけて、米州では死亡リスクと健康寿命の損失が最も増加しました。
その一方で、西太平洋地域では大幅な減少が記録されています。
Between 2019 and 2023, the global age-standardised rate of“healthy life” lost rose by about 20 per cent for depressive disorders and nearly 45 per cent for anxiety disorders.
Fordrug use disorders, the Americas recorded the largest increases in risk of death and healthy life lost between 2000 and 2023, while the Western Pacific saw substantial declines.

最後に、今後の見通しと注視すべき点についてお伝えします。
今回の推計データは、単なる統計的な数値にとどまるものではありません。
死因や病気の負担が、時間の経過とともにどのように変化しているかを可視化するものです。
各国がこの客観的な証拠を活用することで、それぞれの国における保健政策や、医療の優先事項を適切に策定していくためのエビデンスを提供しています。
今後、このデータに基づいて各国の保健システムがどのように整備されていくのか、注視していく必要があります。
“The value of these estimates is not only in the numbers themselves,” Dr. Labrique said. “By showing how causes of death and disease burden are changing over time and across populations,they give countries evidence to help shape health policies and priorities.”
