寿命回復の裏で進む構造変化、WHO最新推計が示す非感染性疾患と精神健康の新たな課題

世界保健機関、通称WHOが、2000年から2023年における世界の健康状態の推移をまとめた最新の「世界保健推計」を公表しました。
この報告書は、新型コロナウイルス感染症のパンデミックによる混乱を経て、世界の健康状況がどのように変化したかを示しています。
世界の平均寿命はパンデミック前の水準近くまで回復したものの、健康寿命の回復は遅れていることが明らかになりました。
同時に、糖尿病や認知症、精神健康状態、薬物使用障害といった非感染性疾患、いわゆるNCDsが死亡原因に占める割合が世界的に高まっています。
今回は、この変化し続ける世界の保健医療の現状について、主要なデータを交えてお伝えいたします。

第一の論点は、世界の平均寿命の回復と、死因の非感染性疾患への長期的な移行です。
2023年の世界の平均寿命は73.3歳に達し、2019年に記録された73.4歳に極めて近い水準まで回復しました。
しかし、健康に暮らすことができる期間を示す「健康寿命」の回復は遅く、2023年は62.8歳にとどまり、2019年の水準を0.4歳下回っています。
また、死因の非感染性疾患へのシフトは続いており、2023年の世界全体の死亡数の74%を非感染性疾患が占め、2000年の58%から大幅に増加しました。
さらに、2023年には低所得国において、初めて感染症などの伝染性疾患による死亡が全体の半分未満となり、世界的な疫学的移行が進んでいることが示されました。
Global life expectancy reached 73.3 years in 2023, close to the 73.4 years recorded in 2019. Healthy life expectancy has recovered more slowly, reaching 62.8 years in 2023, still 0.4 years below its 2019 level. At the same time, the long-term shift towards NCDs continues. NCDs accounted for 74% of deaths globally in 2023, compared with 58% in 2000. Eight of the world’s ten leading causes of death were NCDs in 2023. This shift is increasingly visible across all income levels. In 2023, for the first time, communicable diseases accounted for less than half of all deaths in low-income countries, marking another step in the global epidemiological transition.

第二の論点は、心血管疾患の継続的な負担と糖尿病・認知症の急増です。
心血管疾患は、依然として世界における最大の死因であり、かつ最大の疾病負荷となっています。
2023年には、虚血性心疾患だけで約950万人の死亡が発生し、健康な生活が失われた年数を示す「障害調整生存年」は2億1000万年に達しました。
2000年以降、多くの地域で虚血性心疾患の負担軽減に進展が見られたものの、WHOの西太平洋地域および東南アジア地域では個人レベルでのリスク増加が記録されています。
さらに、他の慢性疾患も顕著になっており、2000年以降、特に東南アジアで糖尿病による死亡リスクが大幅に上昇したほか、認知症による世界全体の死亡数はこの期間に3倍に急増し、2023年には世界第5位の死因となりました。
Cardiovascular diseases remain the leading cause of death and disease burden worldwide. In 2023, ischaemic heart disease alone caused approximately 9.5 million deaths and 210 million disability-adjusted life years (DALYs), measured in years of healthy life lost. There has been substantial progress in reducing the burden of ischaemic heart disease since 2000 in many parts of the world. However, increases in risk at an individual level were recorded in the WHO Western Pacific and South-East Asia Regions, highlighting persistent inequalities in cardiovascular health between populations. Other chronic conditions are also becoming increasingly prominent. The risk of dying from diabetes has risen substantially since 2000, particularly in South-East Asia. Alzheimer disease and other dementias have also risen sharply, for example dementia became the fifth leading cause of death globally in 2023, compared with 19th in 2000. Global deaths from dementia tripled over this period.

第三の論点は、精神健康状態の悪化と薬物使用障害による疾病負荷の増大です。
2019年から2023年の間に、うつ病性障害による年齢調整済みの障害調整生存年の割合は約20%増加し、不安障害では45%近くも増加しました。
これら2つの精神健康状態により、2023年には早期死亡や障害を通じて、推定1億1000万年分もの健康な生活が失われたとされています。
また、薬物使用障害についても地域ごとに異なる傾向が示されており、2000年から2023年にかけて、WHOの米州地域では死亡や健康損失のリスクが最も大きく増加しました。
一方で、西太平洋地域においては、薬物使用障害によるリスクが大幅に減少していることが報告されています。
Between 2019 and 2023, the global age-standardized DALY rate increased by approximately 20% for depressive disorders and nearly 45% for anxiety disorders. Together, these conditions accounted for an estimated 110 million years of healthy life loss through premature death and disability in 2023. Drug use disorders showed similar trends across regions. Between 2000 and 2023, the WHO Region of the Americas recorded the largest increases in risk of mortality and healthy life loss while the Western Pacific saw substantial declines.

最後に、今回の推計がもたらす今後の展望と注視すべき点についてまとめます。
WHOのデータ・デジタルヘルス・アナリティクス・AI部門のディレクターは、寿命が延びたことは公衆衛生の偉大な成果であると指摘しています。
その上で、次の課題は、延びた寿命をいかに健康な状態で過ごせるようにするか、そして変化する人口ニーズに医療システムが対応できるようにすることであると述べています。
この世界保健推計は、死因や疾病負荷が時間とともにどのように変化しているかを示すことで、各国が保健政策や優先事項を策定するための証拠となります。
また、WHOは各国が強靭なデジタル・データインフラを構築し、健康データを分析してタイムリーで実効性のある知見を生み出す能力を高めるための支援を続ける方針です。
今後、これらのデータ分析能力を強化し、ターゲットを絞った効果的な介入を行うことで、すべての人の健康と幸福をさらに向上させることが期待されます。
“Living longer is one of the great achievements of public health,” said Dr Alain Labrique, Director of WHO’s Department of Data, Digital Health, Analytics and AI. “The next challenge is to ensure that those additional years are lived in good health, while health systems are equipped to respond to the changing needs of populations.”

“The value of these estimates is not only in the numbers themselves,” said Dr Labrique. “By showing how causes of death and disease burden are changing over time and across populations, they give countries evidence to help shape health policies and priorities. They also help WHO support countries in building resilient, essential digital and data infrastructure while strengthening national and subnational capacity to analyse health data and generate timely, actionable insights. These capabilities are critical to better enable targeted, effective interventions and ultimately further improving health and well-being for all.”
