急増する子どもの肥満に初の国際指針、WHOが薬物や手術を「最後の手段」とする新ガイドラインを発表

世界保健機関、WHOは、急増する子どもの肥満に対処するため、初となるグローバルガイドラインを発表しました。

1990年以降、子どもや若者の肥満の割合は4倍に増加しており、5歳から19歳のグループでは全体の約8パーセントに達しています。

WHOはこの肥満について、過剰な脂肪蓄積によって引き起こされる「慢性で再発性の疾患」であると定義しています。

Child and adolescent obesity has quadrupled since 1990, rising from two per cent to around eight per cent of youngsters in the five-to-19 age group, according to the UN World Health Organization (WHO).

The UN health agency defines obesity as a “chronic, relapsing disease” triggered by excessive fat deposits which can nonetheless be managed through effective care.

現在、世界では非常に多くの子どもたちがこの病気に直面しています。

2024年のデータによりますと、世界で約1億7000万人の子どもや青少年が肥満を抱えて生活しているということです。

内訳としては、5歳から9歳の子どもが7000万人、10歳から19歳までの青少年が約1億人に上ります。

In 2024, there were around 170 million children and adolescents living with obesity – including 70 million children aged five to nine and some 100 million adolescents up to 19.

Obesity increases the risk of noncommunicable diseases, including type 2 diabetes and cardiovascular disease.

It also increases their likelihood of experiencing stigma, discrimination and bullying, which can strongly negatively affect mental health.

WHOが示した新しいガイドラインでは、食事内容の変更や身体活動、そして行動を促す介入措置を強く推奨しています。

専門家も、子どもたちへの支援において、持続可能な行動変容を可能にする環境づくりが最も重要であると指摘しています。

In its new set of guidelines, WHO strongly recommends dietary changes, physical activity and other behaviour-changing interventions.

“The foundation of obesity care for children and adolescents is not medicine or surgery, but access to comprehensive support that enables healthy eating, physical activity and sustainable behaviour change,” said Dr. Luz María De Regil, Director of the Department of Nutrition and Food Safety at WHO.

主要な論点2:薬物療法や減量手術は「最後の手段」に限定

今回のガイドラインでは、年齢に応じた治療の段階が明確に示されました。

まず、0歳から9歳の子どもに対しては、薬物治療や減量手術、また減量器具の使用は推奨されていません。

10歳から19歳の青少年についても、専門家の指導による生活習慣プログラムで改善が見られなかった場合にのみ、承認された医薬品の検討を行うべきだとしています。

さらに、重度の肥満を抱える青少年に対する減量手術についても、極めて厳格な条件のもとでのみ考慮されるべきであると定められました。

It does not recommend pharmacological treatment, bariatric surgery (surgery that changes the digestive system to induce weight loss) or weight-loss devices for children aged 0-9 years.

For adolescents aged 10-19 years, treatment with approved medicines – such as GLP-1 therapies – should only be considered when a supervised lifestyle programme has failed to make improvements.

Similarly, bariatric surgery may be considered under strict conditions for those with severe obesity.

主要な論点3:メンタルヘルスケアの統合と個別のニーズへの対応

ガイドラインは、肥満のケアにおいてメンタルヘルスへのアプローチを同時に行うことの重要性を強調しています。

不安やうつ、自尊心の低下、感情の調節障害などが、不健康な食事や運動不足、社会的引きこもりを招く原因となるためです。

また、肥満を抱える子どもたちが受けるスティグマやいじめが、精神的な健康をさらに害するという側面もあります。

そのため、単なる体重減少を目的にするのではなく、個々の子どものニーズに合わせた長期的な追跡調査を行い、健康と幸福感を向上させることを求めています。

The new recommendations also emphasise the importance of addressing mental health alongside obesity, because it is both a cause and a consequence of the disease.

Anxiety, depression, low self-esteem and emotional dysregulation can contribute to unhealthy eating behaviours, physical inactivity and social withdrawal, while children and adolescents living with obesity may also experience stigma and bullying that harm their emotional well-being.

The guidelines stress that obesity care should go beyond weight loss and aim to improve health, functioning and well-being, notably by being tailored to the specific needs of each child and by ensuring long-term follow-up.

結び:求められる社会システム全体の変革

WHOは、治療やケアを行うだけでは肥満の増加を食い止めることはできないと指摘しています。

この問題を根本的に解決するためには、社会システム全体の変革が必要不可欠です。

今後は、健康的な食生活や運動を、すべての人がより手軽に、そして安価に利用できるようにするための政策が求められます。

具体的には、規制や財政的な措置をはじめ、教育、都市計画、交通、さらには社会的保護にわたる幅広い分野での取り組みを注視していく必要があります。

At the same time, WHO acknowledged that treatment cannot reverse the rise in obesity by itself: systemic changes are needed.

The agency called for policies and measures that make healthy diets and physical activity more accessible and affordable, including regulatory and fiscal measures and action across education, urban planning, transport and social protection.
