チリが南米初の快挙、犬媒介性狂犬病の排除をWHOが認定

世界保健機関、WHOは、チリを南米で初めて、犬が媒介する狂犬病を公衆衛生上の問題として排除した国に認定しました。

この歴史的な成果は、50年以上にわたりヒトの狂犬病症例が発生していない実績を評価したものです。

また、同国が今後の感染再侵入や伝播を防ぐ十分な能力を維持していることも証明されました。

WHO事務局長は、この成果が公衆衛生への持続的な投資の成果であり、他国にとって貴重な教訓になると祝福しています。

チリの取り組みは、狂犬病によるヒトの死亡ゼロを目指す世界各国に対し、非常に貴重な教訓を提供しています。

Chile has become the first country in South America to receive the World Health Organization (WHO) validation for eliminating dog-transmitted rabies as a public health problem.The recognition highlights more than 50 years of progress without a human case of rabies and confirms the country's capacity to prevent transmission from re-emerging.

"I congratulate the government and people of Chile on this historic achievement," said Dr Tedros Adhanom Ghebreyesus, WHO Director-General. "This validation reflects decades of commitment to protecting communities from rabies and demonstrates the impact of sustained investment in public health. Chile’s success offers valuable lessons for countries working to end human deaths from this preventable disease."

チリで最後に報告された犬媒介性のヒト狂犬病症例は、1972年にまで遡ります。

この半世紀以上にわたる成果は、チリが継続してきた国家的な予防・管理プログラムによるものです。

狂犬病は一度症状が現れるとほぼ100%死亡する極めて危険な感染症ですが、適切なワクチン接種で防ぐことができます。

チリでは、犬に噛まれたり引っ掻かれたりした人々に対し、迅速にワクチンを提供する体制が整えられてきました。

さらに、戸別訪問による犬のワクチン接種キャンペーンや、適切な個体数管理、地域社会への啓発活動が一体となって進められました。

近年では、犬や猫を対象とした無料の集団接種、動物におけるウイルスの監視、そして国境地域での予防活動などがさらに強化されています。

こうした多角的な取り組みにより、チリは狂犬病の発生を防ぐ強固な基盤を築き上げました。

Chile's last reported case of human rabies transmitted by dogs occurred in 1972. The achievement followed sustained national efforts to combat the disease through the timely provision of the vaccine to individuals potentially exposed to the rabies virus, typically through bites, scratches, or contact with saliva from infected or suspected rabid dogs. These efforts were complemented by door-to-door dog vaccination campaigns, targeted dog population management measures, as well as community awareness and education initiatives. Since then, Chile has maintained its National Rabies Prevention and Control Programme, which includes free mass vaccination campaigns for dogs and cats, epidemiological surveillance, strengthened laboratory diagnostic capacity, monitoring of rabies virus circulation in animals, and timely access to post-exposure prophylaxis.

WHOによる排除の検証を受けるためには、極めて厳しい基準をクリアする必要があります。

チリは単にヒトの死亡症例が発生していないことだけでなく、強固な監視体制や検査体制が維持されていることを証明しなければなりませんでした。

同国が提出した詳細な技術文書は、独立した国際専門家グループによって厳格に審査されました。

その結果、チリが再侵入に対処できる十分な能力を備えていることが正式に確認されたのです。

チリの保健大臣は、この認定が自治体や地域の専門チーム、そして住民が一体となって公衆衛生政策に取り組んだ結果であると述べています。

To achieve WHO validation, Chile had to demonstrate not only the sustained absence of human rabies transmitted by dogs, but also that it maintains robust surveillance, laboratory, vaccination, and response systems capable of preventing the disease from re-emerging. Chile submitted a technical dossier documenting compliance with WHO’s validation criteria. The dossier was reviewed by an independent international group of experts convened by PANAFTOSA/PAHO/WHO, which confirmed that the country had met the internationally established requirements. "But this achievement would not have been possible without the joint efforts of municipalities, their zoonosis teams and communities. Without communities, without municipalities and without public policy, a certification such as this cannot be achieved."

チリの成功は、アメリカ地域全体で進められている感染症排除に向けた大きな一歩となります。

同地域では1983年以降、犬媒介性のヒト狂犬病症例が98%以上減少しており、世界をリードする取り組みが行われてきました。

今回の成果は、2030年までに30以上の感染症や関連疾患を排除することを目指す、地域のイニシアチブに大きく貢献するものです。

また、チリにとっては、今回の狂犬病の排除が2つ目の「顧みられない熱帯病」、いわゆるNTDの排除達成となります。

チリは同年にハンセン病の排除についてもWHOから検証を受けており、これはアメリカ地域で初の快挙でした。

これにより、チリは地域における感染症対策の先駆者としての地位を確固たるものにしています。

Chile's validation builds on decades of coordinated action across the Americas, which has made the region a global leader in eliminating dog-transmitted rabies. Since 1983, human cases of dog-mediated rabies in the region have fallen by more than 98%, with only a handful of cases reported each year. Chile’s achievement also contributes to the wider regional goal of eliminating dog-transmitted human rabies as part of PAHO's Disease Elimination Initiative, which aims to eliminate more than 30 communicable diseases and related conditions in the Americas by 2030. Rabies is one of 21 neglected tropical diseases (NTDs), which affect one billion people globally. As of September 2026, 65 countries had eliminated at least one NTD, including seven in the Americas. Chile’s validation for rabies marks its second NTD elimination achievement; earlier this year, it was verified by WHO for the elimination of leprosy, becoming the first country in the Americas to achieve this target.

チリにおける狂犬病の排除は、予防や監視、および医療へのアクセスへの投資が、いかに効果的であるかを示す好例です。

世界全体を見ると、狂犬病は今なおアジアやアフリカを中心に、毎年数万人の命を奪い続けています。

人間、動物、そして環境の健康を一体として捉える「ワンヘルス」アプローチの重要性が、今回の成果を通じて改めて浮き彫りになりました。

The progress made in the Americas stands in contrast to the global picture. Dog-transmitted rabies still causes tens of thousands of deaths each year, primarily in Asia and Africa. Chile's success demonstrates that sustained investment in prevention, surveillance and access to care can bring this deadly but preventable disease to the point of elimination. It also illustrates the value of the One Health approach, with human, animal and environmental health sectors working together to prevent and control diseases that spread between animals and people.
