世界の小児がん生存率に初の共通指標、WHOが国別比較データを公開し格差是正へ
世界保健機関、いわゆるWHOは、194カ国を対象とした小児がん生存率のデータを公開しました。
これは、国ごとに比較可能な形でまとめられた初めてのデータとなります。
今回は、小児および思春期のリンパ性白血病における5年生存率の推計値が示されました。
完全なデータセットは、WHOの「世界保健観測所」のウェブサイトから誰でも閲覧することができます。
生存率の向上や、必要な支援を特定するための重要な土台が整ったと言えます。
WHO’s Global Initiative for Childhood Cancer (GICC) published a report on childhood cancer survival, presenting the first country-comparable five-year survival estimates for children and adolescent lymphoid leukaemia across 194 countries. The complete dataset behind these findings is now available online through theWHO Global Health Observatory.

今回のデータ整備により、世界中の関係者が同じ基準で進捗を確認できるようになりました。
これまでは共通の測定基準がなかったため、国際的な比較が困難だったのです。
この新しい指標は、どの地域に優先的な投資が必要であるかを明確にする役割を果たします。
WHOと各国政府は、2030年までに小児がん生存率を世界全体で少なくとも60%に引き上げるという目標を掲げています。
その誓約が守られているかどうかを監視するための、強力なツールが誕生したと言えるでしょう。
This is the first time survival estimates have been produced using a common methodology across all WHO Member States, giving stakeholders a standardized baseline to track progress, compare performance and identify areas where investment is most urgently needed. The data allow WHO and governments to monitor commitments to achieve the WHO’s GICC target of raising childhood cancer survival to at least 60% globally by 2030.

それでは、今回の報告から明らかになった重要な3つの論点について解説します。
まず1つ目は、国や地域の間で生存率に極めて大きな格差が存在しているという事実です。
毎年、世界では推計40万人の子どもや思春期の若者ががんを発症しています。
小児がんの多くは治療可能であるにもかかわらず、生存率は患者が住んでいる場所に大きく左右されるのが現状です。
高所得国では生存率が80%から90%に達している一方で、医療体制が脆弱な地域では40%近くにとどまっています。
特に、西太平洋地域や欧州地域で高い生存率が示された一方、アフリカや東地中海の一部地域では厳しい状況が続いています。
これらの地域では、適時の診断や必須医薬品へのアクセス、質の高いケアの提供において依然として課題を抱えています。
Each year, an estimated 400 000 children and adolescents develop cancer. While most childhood cancers are highly treatable, survival still depends heavily on where a child lives. The data show that high-income countries consistently demonstrate strong performance, with survival rates now reaching 80–90%, while settings with weaker health systems show markedly lower survival, closer to 40%. The WHO Western Pacific and European Regions have reached some of the highest estimated survival levels, while the African Region and parts of the Eastern Mediterranean Region face persistent challenges linked to inequities in timely diagnosis, access to essential medicines and high-quality care.

2つ目の論点は、15歳から19歳の思春期世代における生存率の格差が広がっている点です。
この世代は、14歳以下の子どもたちと比べて生存率の格差が大きくなっています。
特に低中所得国において、その傾向が顕著に見られます。
背景には、診断の遅れや小児科から成人科への移行期における支援の不足、さらに思春期のがん治療に関する証拠の少なさなどが挙げられます。
また、思春期世代は幼い子どもたちとは異なる種類のがんを発症するため、異なる治療経路が必要とされていることも明らかになりました。
The report also shows that adolescents aged 15–19 years face a wider survival gap than children aged 0–14 years, with the divide most pronounced in low- and middle-income countries (LMIC). Long-standing barriers includes delays in diagnosis, gaps between paediatric and adult services, and limited evidence on adolescent cancer care. Adolescents also face different types of cancer than younger children, requiring different care pathways.

3つ目の論点は、がん登録データの不足と、それによる高い未診断率です。
現在、世界人口の約80%が、地域ベースのがん登録制度でカバーされていません。
さらに一部の地域では、がんを発症した子どもの最大43%が、診断も治療も受けられていない実態があります。
正確なデータがないために、適切な政策決定や医療支援が行き届かない悪循環が生じています。
この課題を解決するため、WHOや国際がん研究機関などは専門人材の育成を進め、データの質向上に乗り出しています。
これまでに27の地域で125人の医療専門家が訓練を受けました。
Nearly 80% of the world’s population is not covered by population-based cancer registry. In some regions, up to 43% of childhood cancer cases go undiagnosed and untreated. To strengthen cancer registration and data quality,the Global Initiative for Cancer Registry Development(ChildGICR) and WHO’s International Agency for Research on Cancer (IARC) have trained 125 health professionals across 27 territories.

最後に、今後の展望と注視すべき動きについてお伝えします。
2025年には、2030年までに生存率を60%に引き上げるという世界目標が、国連総会の政治宣言に正式に盛り込まれました。
これにより、国際社会の政治的な意思がより強固なものとなっています。
適切な計画策定と継続的な投資が行われれば、2030年までにさらに100万人の命を救うことが可能になると期待されています。
2018年の取り組み開始以降、これまでに87カ国が参加し、400以上のプロジェクトが実施されてきました。
すでに100カ国以上で小児がんサービスの国家評価が完了しており、50カ国が国家がん管理計画に小児がん対策を組み込んでいます。
さらに、約20カ国が国内資金を確保することに成功しました。
今後も各国の具体的な資金確保や、国家計画の進捗状況を注視していく必要があります。
In 2025, the global target of 60% childhood cancer survival by 2030 was formally included in the political declaration of the United Nations General Assembly's high-level meeting on noncommunicable diseases, demonstrating unpreceded global political commitment. With coordinated childhood cancer control planning, sustained political commitment and strategic investment, the ambition of saving 1 million additional lives by 2030 is within reach. By 2024, more than 100 Member States had completed national assessments of childhood cancer services, and 50 countries across all six WHO regions had incorporated childhood cancer into national cancer control plans. With WHO’s support in sustainable long-term financing planning under the framework, around 20 countries have successfully secured domestic funding for childhood cancer programmes.
