妊娠中の高血糖に関する診断基準をWHOが改訂へ、2026年10月に専門家会合を開催

世界保健機関、通称WHOが、妊娠中の高血糖に関する診断基準と分類の推奨事項を改訂することを発表しました。

現在、世界では年間2100万人の出生、つまり6人に1人の割合で、妊娠中の高血糖による影響が発生しています。

妊娠中の高血糖には、妊娠前からの糖尿病や、妊娠中に初めて診断された糖尿病、そして妊娠糖尿病が含まれます。

これらを正確に見分けることは、特に診断されていない糖尿病患者が多い地域において、非常に難しい課題となっています。

妊娠中の高血糖は、妊娠高血圧症候群や死産、巨大児、新生児低血糖などのリスクを高めることが知られています。

そのため、早期に発見し、正しく分類することが、適切なケアを提供しリスクを減らすために極めて重要です。

One in six live births(21 million per year) is affected by hyperglycaemia during pregnancy. Hyperglycaemia in pregnancy includes diabetes that existed before pregnancy, diabetes first diagnosed during pregnancy, or gestational diabetes mellitus (GDM), a form of elevated glucose (blood sugar) levels that appears during pregnancy. Distinguishing between these conditions is often challenging, particularly in settings with a high prevalence of undiagnosed pre-existing diabetes. Hyperglycaemia during pregnancy is associated withincreased risks of adverse maternal and perinatal outcomes, including hypertensive disorders of pregnancy, stillbirth, macrosomia, neonatal hypoglycaemia, birth injury, and other long-term complications. Early identification and accurate classification of hyperglycaemia during pregnancy are therefore critical to enable appropriate care and reduce the risk of adverse outcomes.

世界的に糖尿病の負担は増大しており、その多くは低所得国や中所得国に集中しています。

多様な医療資源の環境に対応できるガイドラインが求められる中、WHOは2025年11月に、妊娠中の糖尿病女性のケアに関する初めての推奨事項を公表しました。

今回の改訂は、世界中の母子の健康状態を改善するための取り組みの一環として位置づけられています。

Given the substantial and growing global burden of diabetes, with most cases occurring in low- and middle-income countries, guidance that is applicable across diverse resource settings is needed. With a view to improving maternal and newborn outcomes worldwide, the World Health Organization (WHO) published its firstrecommendations on care for women with diabetes during pregnancyin November 2025.

ここからは、今回のニュースにおける主要な論点を3つに分けてお伝えします。

1つ目の論点は、2013年に作成された推奨事項の更新についてです。

2026年に発表される予定の新しい推奨事項では、妊娠中に初めて検出された高血糖の診断基準と分類が見直されます。

The updated recommendations set to come out in 2026 will include updates on the 2013recommendations on the diagnostic criteria and classification of hyperglycaemia first detected during pregnancy. New evidence has emerged regarding approaches to screening, diagnosis, and classification, highlighting the need to revisit this guidance. Revising these recommendations has been prioritized as part of WHO’s broader efforts to address non-communicable diseases in pregnancy and improve maternal and newborn health outcomes worldwide.

2つ目の論点は、ガイドライン開発グループによるエビデンスの検証です。

2026年10月20日から24日にかけて、専門家による会合が開催される予定となっています。

この会合において、妊娠中の高血糖のスクリーニングおよび診断に関する最新のエビデンスが精査されます。

A Guideline Development Group (GDG) meeting will be held 20–24 October 2026 to review the evidence base on the screening and diagnosis of hyperglycaemia in pregnancy.

3つ目の論点は、意思決定プロセスの透明性と利益相反の管理についてです。

WHOは、ガイドライン開発グループに参加するメンバーの略歴をオンラインで公開しています。

メンバー全員が利益相反の申告書を提出しており、学術的、財務的、あるいはその他の重大な利害関係がないことが確認されています。

これにより、ガイドラインの信頼性を担保するための透明なプロセスが徹底されています。

In keeping with the requirements of the WHO Compliance, Risk Management and Ethics Office,we are posting online short biographies of the GDG members. The listed candidates have also submitted a declaration of interest form stating any conflict of interests. WHO has applied its internal processes to ensure that the performance of the above tasks by members of this group will be transparent and without any significant conflict of interests (academic, financial or other) that could affect the credibility of the guideline.

最後に、今後のスケジュールと注視すべき点についてお伝えします。

WHOは一般の人々に対しても、公開された専門家の経歴を確認し、利益相反に関する意見やフィードバックを送るよう呼びかけています。

この専門家会合は完全な招待制であり、参加する専門家は個人の立場で意見を述べることになります。
