妊産婦死亡の主要原因に挑む、WHOが「妊娠期高血圧疾患」対策の国際ロードマップを発表
世界保健機関(WHO)とパートナーは、妊産婦や新生児の命を脅かす妊娠期高血圧疾患に対処するため、新たなグローバルロードマップを発表しました。
この疾患は子癇前症などを含み、世界中で毎年数百万人の女性に深刻な影響を与えています。
今回のロードマップは、2026年から2035年以降を見据え、研究や医薬品へのアクセス改善など、包括的な行動計画を示すものです。
The World Health Organization (WHO) and partners today launched anew global roadmapto tackle hypertensive disorders of pregnancy, includingpre-eclampsia, which affect millions of women every year. These disorders remain among the leading causes of death and serious illness for pregnant women and their babies, with tragic long-term impact on their families.

1.妊娠期高血圧疾患による高い死亡率と、基礎的な医療アクセスにおける深刻な格差
妊娠期高血圧疾患は、世界中の妊娠の10%から15%に合併すると推定されています。
これは世界の妊産婦死亡の約16%を占め、毎年約42000人の妊産婦の命を奪っているのが現状です。
さらに、年間50万人以上の死産や新生児死亡にもつながっています。
多くの国では、基本的な血圧測定や尿蛋白検査、あるいはアスピリンや硫酸マグネシウムなどの必須医薬品へのアクセスがいまだに不足しています。
搬送体制の不備や訓練された医療従事者の不足、限られた緊急産科サービスも、救命のためのケアを遅らせる要因となっています。

Hypertensive disorders complicate an estimated 10–15% of pregnancies worldwide. They account for around 16% of maternal deaths globally, resulting in an estimated 42 000 deaths each year, as well as more than 500 000 stillbirths and newborn deaths. In many countries, women still lack reliable access to basic blood-pressure measurement, urine protein testing and essential medicines, including aspirin, antihypertensives and magnesium sulfate. Weak referral systems, shortages of trained health workers and limited emergency obstetric services can further delay life-saving care. “Pre-eclampsia can develop in any pregnancy. Whether it becomes fatal depends on whether a woman's blood pressure was checked in time, and whether quality emergency care was within reach when it turned severe,” said Dr Pascale Allotey, Director of WHO’s Department of Sexual, Reproductive, Maternal, Child, Adolescent Health and Ageing.

2.2026年から2035年以降に向けた「グローバルロードマップ」の策定と優先研究分野の特定
妊産婦・胎児医学会のグローバルカンファレンスで発表されたロードマップは、研究、臨床ガイドライン、医薬品と診断へのアクセス、実施、および説明責任における協調的な行動を定めています。
特に、子癇前症などの予防や治療に特化し、規制当局に承認された新規医薬品の開発が進んでいないというイノベーションのギャップが強調されました。
これまでの研究投資は診断技術に偏っており、新たな予防や治療の選択肢への投資が慢性的に不足しています。
この課題を解決するため、WHOは2026年5月にグローバルサミットを開催し、140以上のパートナーとともに、今後の投資を導く20の優先研究質問を特定しました。
Launched at the Society for Maternal-Fetal Medicine Global Conference, theGlobal roadmap for hypertensive disorders of pregnancy from 2026 to 2035 and beyondsets out coordinated actions across research and innovation, global clinical guidance, access to medicines and diagnostics, implementation, advocacy and accountability. The roadmap highlights a striking innovation gap between the scale of the problem and the lack of medicines specifically developed and approved by regulators for the prevention or treatment of pre-eclampsia or eclampsia. Research investment has largely focused on diagnostics, with chronic underinvestment in new preventive and treatment options. To change this, WHO brought together more than 140 partners at aglobal summit in May 2026to identify and answer 20 priority research questions to guide future investment that will help predict, detect and treat hypertensive disorders in pregnancy.

3.母子における出産後の長期的な健康リスクと社会的な脆弱性
妊娠期高血圧疾患の影響は、出産直後にとどまらず、母子の生涯にわたる健康リスクとなることが示されています。
罹患した女性は、将来的に慢性高血圧や心血管疾患、脳卒中、腎臓病を発症するリスクが高まります。
また、胎内でこの疾患にさらされた子どもについても、早産に伴うリスクに加え、長期的な心血管疾患や代謝性疾患のリスクが上昇することが明らかになっています。
さらに、貧困、人道危機、地理的孤立などの課題に直面している女性は、特に高いリスクに直面していることが指摘されています。
Pre-eclampsia can also have consequences for mothers long after childbirth. Women affected by hypertensive disorders of pregnancy face increased risks later in life of chronic hypertension, cardiovascular disease, stroke and kidney disease, while children exposed during pregnancy can face increased risks associated with preterm birth and longer-term cardiovascular and metabolic disease. In low- and middle-income countries, gaps in antenatal care, medicines, diagnostics, trained health workers and emergency services mean women are more likely to develop severe complications or die. Women affected by poverty, humanitarian crises, geographic isolation and other challenges face particularly high risks.
