施設隔離から地域での生活へ、WHOが精神保健ケアの転換を目指す新ガイドを発表

世界保健機関(WHO)は、世界精神保健デーに向けて新たなガイドを発表しました。

このガイドは、長期間の精神科病院への入院を中心とした施設型ケアから、本人の権利と選択を尊重する地域社会での個人中心ケアへの移行を各国で支援する目的で作成されています。

現在、多くの精神疾患を持つ人々が大規模型の精神科病院や社会福祉施設で生活を送っています。

そのうち約半数が、数ヶ月から数年、さらには数十年にもわたって意に反して収容されており、外部の世界から孤立している状況にあります。

WHOはこうした施設依存のモデルからの脱却を目指し、持続的な投資の転換を強く促しています。

Ahead of World Mental Health Day, the World Health Organization (WHO) has released a newguide to support countries in shifting away from institutionalized mental health care models focused on long-term stays in psychiatric hospitals towards person-centred care in the community that respect people’s rights and choices.

Many people with mental health conditions are currently living in large psychiatric hospitals and social care institutions. Nearly half are kept in these facilities involuntarily for months, years, or even decades, isolating them from the outside world.

1. 「脱施設化」に向けた実践的ガイドの提示と包括的支援の重視

「脱施設化」とは、精神科病院に6ヶ月以上の長期間滞在した人々が地域社会に戻り、充実した生活を送るために必要なケアや支援、機会を得るプロセスを指します。

WHOが新たに作成したガイドでは、精神科病院のリーダーがこの「脱施設化」を実践するための具体策が示されています。

具体的には、病院への新規入院や長期滞在の削減、利用者の権利を守るための施設内慣行の変更、そして必要な支援を受けながら地域へ復帰するための手順が盛り込まれました。

また、この取り組みは病院の力だけで達成できるものではなく、住宅、医療、社会的支援、教育、雇用機会、家族への支援といった多角的な環境整備が欠かせないと示されています。

Known as deinstitutionalization (DI), this process involves helping people who have spent long periods (six months or more) in psychiatric hospitals to return and live in their communities, with the care, support and opportunities they need to lead fulfilling lives.

The new guide sets out practical actions for psychiatric hospital leaders to implement DI. It focuses on reducing psychiatric hospital admissions and long stays, changing institutional practices to better protect people’s rights, and helping people leave hospital and return to community life with the support they need.

It also makes clear that DI cannot be achieved by hospitals alone. Access to housing, health care, social support, education, employment opportunities and support for families are essential to helping people live well in their communities.

2. 世界的な改革進捗の遅れと地域移行の偏り

多くの国で地域ベースの精神保健サービスを強化する動きが見られるものの、全体的な進捗には大きな偏りが見られます。

WHOの調査データ「Mental Health Atlas」によると、地域社会を中心とした精神保健ケアへ完全に移行できていると報告した国は、全体のわずか9パーセントにとどまっています。

一方で、半数を超える53パーセントの国がいまだ改革の初期段階にあることが明らかになりました。

また、精神科病院に入院した人のうち4人に1人以上にあたる28パーセント超が、6ヶ月を超える長期滞在となっている事実も報告されています。

Many countries are working to strengthen community-based mental health services, but progress remains uneven. WHO’sMental Health Atlasfound that only 9% of reporting countries had fully shifted to community-based mental health care, while more than half (53%) were still at an early stage of reform. 3. 当事者の経験を中心に据えた政策・サービス設計への転換

新しいガイドは、今年の世界精神保健デーのテーマである「当事者の体験に耳を傾ける:リアルな声、リアルな変化」の理念に沿って策定されました。

実際に施設ケアを経験した当事者が、自らの生活に影響を与える政策やサービス、意思決定のプロセスにおいて能動的なパートナーとして参加する重要性が強調されています。

これまで制度の設計や改革の決定から当事者が除外されがちであった現状を改め、回復や地域参加に必要な支援を理解するための貴重な知見として当事者の専門性を活用することが求められています。

単に当事者の「ために」構築するのではなく、当事者と「共に」精神保健システムを作り上げることが重要視されています。

The new DI guide aligns with this year’sWorld Mental Health Daytheme, “Lived experiences heard: real voices, real change,” and highlights the importance of ensuring that people with lived experience are active partners in shaping the policies, services and decisions that affect their lives.

Too often, people who have experienced institutional care are left out of decisions about how mental health systems are designed and reformed – decisions that directly shape the care, treatment and support they receive. Yet their knowledge and experience are vital to understanding what helps people recover, take part in community life, and access the services and support they need.

WHOは世界精神保健デーにあわせ、各国政府やサービス提供機関に対して、施設中心から地域中心の精神保健ケアへの移行を加速させるよう呼びかけています。

今回発表されたガイドは、世界中の専門家や当事者の参画によって作成されており、今後は各国の状況や地域の文脈に応じた実効的な適用が注視されます。

権利に基づいた精神保健システムの構築に向け、資金投入のシフトや住居・雇用などの社会基盤整備がどのように具体化されるかが今後の重要な議論となる見込みです。

The guide was developed with experts from around the world, including people with lived experience. Its practical approaches can be adapted to different countries and local contexts.

On World Mental Health Day, WHO is calling on governments and services to accelerate the shift from institutional to community-based mental health care and build mental health systems together with people with lived experience, not simply for them.
