国際薬剤師会（FIP）は、2026年9月1日に新たな政策声明を公表しました。

この声明は、防ぐことが可能な医薬品関連の危害が依然として世界中で数百万人に影響を与えている現状に対処するためのものです。

薬剤師が持つ専門知識を患者の安全確保に最大限に活かすよう、各国に強く働きかけています。

Medication-related harm continues to affect millions of people worldwide despite being largely preventable, prompting the International Pharmaceutical Federation (FIP) to call for greater recognition of pharmacists' leadership in patient safety through a newStatement of Policyreleased today.

まず1つ目の変更点は、WHOの国際的な取り組みとの整合性を高め、2020年の旧方針を最新の状況に合わせて更新したことです。

今回の政策声明は、世界保健機関（WHO）による「薬害のない社会」イニシアチブや「世界患者安全行動計画2021–2030」を基盤として作成されました。

新しいエビデンスや技術革新、そして進化するケアモデルを反映し、薬剤師が果たすべき役割を再定義しています。

政策委員会の共同議長は、薬剤師がリスクを特定し、薬物使用を最適化する特別な訓練を受けていると指摘しました。

それにもかかわらず、国の患者安全戦略において薬剤師の専門知識が十分に活用されていない現状を打破することが求められています。

Building on the World Health Organization's (WHO) Medication Without Harm initiative and Global Patient Safety Action Plan 2021–2030, the updated policy highlights pharmacists' essential role in preventing medication errors, reducing avoidable harm and improving the safe use of medicines across every stage of the patient's healthcare journey. It also replaces FIP's 2020 policy statement on patient safety and reflects new evidence, technological advances and evolving models of care.

"Pharmacists are uniquely trained to identify risks, optimise medicines use and intervene before harm occurs. Yet too often their expertise is underused within national patient safety strategies. This policy calls for pharmacists to be recognised as leaders in creating safer healthcare systems,"

次に2つ目の変更点は、薬剤師の役割が単なる「調剤ミスの防止」にとどまらないことを明確にした点です。

薬剤師は、薬物療法のあらゆるプロセスにおいて貢献することが求められています。

具体的には、持参薬の確認を行う処方薬の照合、ハイリスク薬の管理、多剤併用であるポリファーマシーの削減が挙げられます。

さらに、医薬品安全性監視の強化や、医療機関の移行期におけるケアの改善、患者への安全な薬の使用に関する教育も重要な役割として位置づけられました。

The statement emphasises medication safety extends beyond preventing dispensing errors. Pharmacists contribute throughout the medication-use process by conducting medicines reconciliation, supporting high-risk medicines management, reducing polypharmacy, strengthening pharmacovigilance, improving transitions of care and educating patients to use medicines safely.

そして3つ目の変更点は、人工知能（AI）などのデジタル技術の導入と、薬剤師の専門的判断との関係性を整理したことです。

電子処方箋や臨床決定支援システム、AIといったデジタル技術は、患者の安全性を大幅に向上させる可能性を秘めています。

しかし、不適切に導入された場合には新たなリスクを生み出す懸念も指摘されています。

そのため、技術は薬剤師などの医療従事者による専門的な判断を代替するものではなく、あくまで補完するものであるべきだと強調されました。

Recognising the changing landscape of healthcare, the policy also highlights the growing role of digital technologies, including electronic prescribing, clinical decision support systems and artificial intelligence. While these innovations have the potential to improve patient safety significantly, they can also introduce new risks if poorly implemented. FIP stresses that technology should complement—not replace—the professional judgement of pharmacists and other healthcare professionals.

今後のスケジュールや注視すべき点について説明します。

今回の声明では、世界の医薬品安全を強化するため、各国政府や医療システムに対して具体的な行動を求めています。
