【国連・WHO報告】「健康な高齢化の10年」中間報告が示す進展と、世界に立ちはだかる資金・データの壁
国連総会に、高齢者の生活向上を目指す「健康な高齢化の10年」の中間進捗報告書が提出されました。
この報告書は、世界保健機関(WHO)をはじめとする多くの国連機関や世界銀行などが共同で作成したものです。
世界の60歳以上の人口は、2020年に10億人を超え、2050年には21億人に達すると予測されています。
しかし、寿命が延びたからといって、必ずしも健康な生活が送れているわけではありません。
2000年から2023年の間に、世界的に平均寿命と健康寿命の差は広がっています。
WHOの高齢化部門の担当者は、前半の5年間で得られた成果をもとに、後半はさらなる規模拡大と資金投入が必要であると述べています。
The Secretary-General transmitted to the 81st session of the UN General Assembly the mid-term progress report on the UN Decade of Healthy Ageing (2021–2030), marking the midpoint of the ten-year global effort to improve the lives of older people. The report was prepared by the World Health Organization (WHO) in coordination with the UN Inter-Agency Group on Ageing and with inputs from thirteen UN entities, the World Bank Group and civil society partners. ... “The first half of the Decade has shown what is possible. 主要な論点の一つ目は、世界各国における高齢者向けの法制度や国家政策の整備が進んでいることです。
年齢に基づく差別を禁止する法律を持つ国は、2018年の44.5パーセントから、現在は57.7パーセントに増加しました。
また、長期ケアに関する国家政策を策定している国も、2023年の49パーセントから56.7パーセントへと上昇しています。
さらに、高齢者に優しい環境づくりのためのプログラムを持つ国や、総合的な保健・社会評価を実施している国もそれぞれ半数を超えました。
WHOのガイドラインに沿って、コミュニティでの統合ケアや長期ケアを国のユニバーサル・ヘルス・ケアのパッケージに組み込む国が増えています。
The report finds that more countries than ever now have legislation against age-based discrimination – up from 44.5% in 2018 to 57.7% today – and over half now have national long-term care policies, up from 49% in 2023 to 56.7%, dedicated programmes to build age-friendly environments (54.6%), and comprehensive health and social assessments of older people (54.6%). New WHO guidance on services for long-term care and for Integrated Care for Older People (ICOPE) delivered in communities is enabling systemic change as an increasing number of countries incorporate these services within national universal health care benefit packages.

主要な論点の二つ目は、深刻な資金不足がこの取り組みにおける最大の制約になっていることです。
報告を行った国の3分の1以上にあたる33.8パーセントにおいて、長期ケアを開発するための専用予算がほとんどない、あるいは全くない状況にあります。
他の優先分野でも同様の資金不足が見られます。
また、高齢者が自立して尊厳ある生活を送るために不可欠な、補聴器やメガネ、歩行器などの自助具へのアクセスを保障する法律を持つ国は、全体の45.4パーセントにとどまっています。
Financing remains the binding constraint: more than a third of reporting countries (33.8%) have very little or no dedicated resources to develop long-term care, with similar shortfalls across the Decade’s other priority areas. Less than half of countries (45.4%) have legislation providing access to assistive devices (including hearing aids, spectacles or rollators), which are key to enabling older people to maintain productive, independent and dignified lives.

主要な論点の三つ目は、データの追跡能力の不足と、関係者間の対話の減少です。
高齢者の健康状態を長期的に追跡・把握できる国はわずか22.7パーセントにとどまり、今回の報告書における指標の中で最大のギャップとなっています。
高齢者それぞれの異なるニーズに対応した政策を設計するためには、各国独自の代表的な情報が必要です。
一方で、高齢化に関するマルチステークホルダー・フォーラムが活動していると報告した国は、2020年の53.1パーセントから、現在は43.3パーセントへと減少しており、対話の場が後退している実態が明らかになりました。
Only 22.7% of countries can track older people’s health over time – the largest gap across the Decade’s core progress indicators. Nationally representative information is needed in each country, as policies and interventions cannot be designed as if all older adults of a given age share similar needs and capacities. ... progress is uneven and in one area, regressive: fewer countries report active multi-stakeholder forums on ageing, down from 53.1% in 2020 to 43.3% today.

最後に今後の見通しと注視すべき点についてお伝えします。
報告書は、2030年までにこれらすべての課題を解決するため、国家予算の確保や開発協力、革新的な資金調達メカニズムを通じた持続的な投資を呼びかけています。
世界銀行グループの担当部門は、健康な高齢化への投資は単なるコストではなく、政府にとって非常に投資対効果の高い選択肢であると強調しています。
統合ケアや長期ケアに早期に投資する国では、初期費用を大幅に上回る削減効果が得られることが、本報告書でも示されています。
今後は、各国政府が予算を確保し、具体的な計画を実行に移せるかどうかが焦点となります。
