難民・移民の健康を守る「能力基準」を実践へ:WHOなどがローマで国際会議を開催

2026年9月21日から24日にかけて、イタリアのローマにおいて、第2回となる「難民・移民の健康に関する政策立案者向けグローバル・オリエンテーション・コース」が開催されました。

この重要な会議には、7つの国や地域から、医療政策の立案者、保健マネージャー、そして技術専門家たちが一堂に会しました。

共同で主催したのは、世界保健機関(WHO)の健康と移民プログラム、およびイタリア国立移民貧困健康研究所(INMP)です。

本プログラムでは、保健システム全体における能力を強化することが、難民や移民の健康状態の向上にどのように寄与できるかが深く探求されました。

From 21 to 24 September 2026, policy-makers, health managers and technical experts from seven countries and territories gathered in Rome for the second Global Orientation Course for Policy-makers on Refugee and Migrant Health. Jointly organized by WHO Health and Migration Programme (PHM) and the Italian National Institute for Health, Migration and Poverty (INMP), the programme explored how strengthening competencies across health systems can contribute to improved health outcomes for refugees and migrants.

世界の公衆衛生において、移民や強制移動への対応は避けて通れない優先課題となっています。

これに伴い、保健システムには、多様な人々のニーズに応えるための包括的な政策や効果的なガバナンスが求められています。

さらに、すべての人が公平で文化に配慮した、人間中心のケアを受けられるよう、医療従事者の体制を整えることも重要です。

今回の研修は、2024年に実施された第1回の成果を引き継ぎ、各国が「WHO世界能力基準」を導入し、国内の対応力を高めるための具体的な支援を行うことを目的として進められました。

As migration and displacement continue to shape public health priorities, health systems require the capacity to respond to diverse population needs through inclusive policies, effective governance and a workforce equipped to deliver equitable, culturally responsive and people-centred care.

主要な論点・合意事項

1.WHO世界能力基準の各国における適応と人材育成の推進

今回の研修における中心的なテーマは、WHOが提示する「世界能力基準」を、それぞれの国や地域の状況に合わせて具体的に適用することでした。

参加者たちは、各地で異なる移民の移動パターンや、それぞれの国の医療制度、政策の優先順位に基づき、基準をどう現地化するかについて議論を行いました。

INMPの総局長は、医療の公平性を高めるためには平等の原則を実践に移すことが不可欠であると述べています。

さらに、医療従事者の能力を高めることこそが、すべての人に包括的で質の高い医療を届ける基盤になると指摘しました。

WHOの責任者もまた、国際的な約束を各国の現場での実践的な行動へと翻訳することが、難民や移民、そしてそれを受け入れる地域社会の健康格差を縮小するために極めて重要であると訴えました。

A central theme throughout the course was contextualization of the WHO Global Competency Standards to different displacement and migration patterns, health system structures and policy priorities. Discussions highlighted shared challenges, innovative approaches and the value of learning and collaboration across contexts.

2.多セクター連携と地域的な医療アクセス調整の強化

2つ目の重要な論点は、保健当局と地域社会のパートナー、あるいは異なる省庁間での連携体制を強化し、医療アクセスを向上させることです。

会議では、いくつかの国から非常に先進的な事例が紹介されました。

例えばアルバニアでは、国家移民戦略に沿って保健当局と地域社会のパートナーとの協調を深め、難民や移民のための医療継続性や紹介経路を強化しています。

コンゴにおいては、省庁間の調整や人道支援パートナーとの協力体制を推進し、難民を予防接種などの保健サービスに統合する試みが進められています。

またコソボでは、難民や移民が精神保健サービスを含む必要な医療を継続して受けられるよう、調整された紹介経路と統合された保健サービスを強化しています。

enhancing coordination between health authorities and community partners, in line with the national migration strategy and action plan, to strengthen access, continuity of care and referral pathways for refugees and migrants in Albania;

promoting interministerial coordination and collaboration between national authorities and humanitarian partners to support refugee inclusion in health services in Congo, including vaccination campaigns and tailored multilingual health communication during health emergencies;

strengthening coordinated referral pathways and integrated health services for refugees and migrants in Kosovo*, supporting access to care, mental health services and continuity of care;

3.エビデンスに基づく政策立案と国境・地域社会の準備体制の構築

3つ目の論点は、評価データなどの科学的根拠に基づいた政策立案を行うこと、そして国境や地域社会における実務的な対応力を高めることです。

リビアでは、国際保健規則(2005)に準拠し、保健分野とそれ以外の分野との連携を深め、国境を越えた難民・移民の健康管理の調整に力を入れています。

リトアニアにおいては、保健システムのレビューから得られた証拠を活用し、難民向けサービスにおける課題や隙間を特定しています。

これにより、政策の開発や資源の適切な配分、サービスの計画立案を効果的に進めています。

タンザニア連合共和国では、国境地域での保健準備体制の整備、最前線で働くスタッフの育成、そして地域社会に根ざしたアプローチを通じて、移動する人々とホストコミュニティ双方の医療ニーズに対応する能力を高めています。

strengthening collaboration across health and non-health sectors and enhancing cross-border coordination for refugee and migrant health in Libya, in line with the International Health Regulations (2005);

leveraging health system review evidence to identify gaps in refugee and migrant health services and inform policy development, resource allocation and service planning in Lithuania; and

strengthening migration health capacities in the United Republic of Tanzania through border health preparedness, frontline workforce development and community-based approaches that support the health needs of people on the move and host communities.

結び

研修の最後には、WHOやINMP、および参加した国や地域の代表から、能力開発に基づいたアプローチが政策を実行に移すために不可欠であるとの見解が示されました。

難民や移民の健康を確保することは、単独の課題ではなく、保健システム全体を強化するための極めて重要な要素です。

今後は、すべての人が適切な医療を受けられる「ユニバーサル・ヘルス・カバレッジ」の達成に向けて、指導力の育成や人材開発への継続的な投資が注視されます。

各国がこの研修で得た知見を持ち帰り、自国の医療政策や現場の体制にどのように反映させていくかが、これからの重要なポイントとなります。

Reflections from WHO, INMP and participating countries and territories highlighted the importance of competency-based approaches in translating policy commitments into action. The course reinforced the role of refugee and migrant health as an integral component of health system strengthening and emphasized the need for continued investment in leadership, workforce development and knowledge exchange to advance universal health coverage.
